(when a background check may be a factor)
Dear Applicant:
This letter is to notify you that we may be unable to make you an offer of employment based on our hiring criteria, including information received from A Matter of Fact (a consumer reporting agency).
Attached is a copy of "A Summary Of Your Rights Under The Fair Credit Reporting Act" and a copy of the report received from A Matter of Fact. You have a right to obtain a copy of the report directly from A Matter of Fact if you make a request in writing to A Matter of Fact within 60 days.
A Matter of Fact did not make any hiring recommendations nor did A Matter of Fact make the hiring decision and is unable to provide specific reasons for the decision.
You have the right to dispute the accuracy or completeness of any information contained in A Matter of Fact’s report by contacting them in writing. Please mail A Matter of Fact the following:- your full name
- mailing address
- daytime telephone number
- social security number
- a photocopy of your "proper identification" -- "Proper identification" includes documents such as a valid driver's license, social security card, military identification card, and credit cards.
- a brief description of your claim of inaccuracy in the report
- copies of documents which support your claim
- names and phone numbers of persons who can substantiate your claims
